Štandardizované hodnotenie nálezov a hlásení zo zobrazovacieho vyšetrenia prsníkov (BI-RADS) je klasifikačný systém, ktorý navrhla The American College of Radiology (ACR) v 80. rokoch minulého storočia s cieľom zmeniť nedostatočnú štandardizáciu a jednotnosť mamografických nálezov v praxi. Súčasťou systému BI-RADS je lexikón, slovník deskriptorov špecifických nálezov zo zobrazovacích vyšetrení. Článok opisuje zmeny v systéme BI-RADS počas jeho používania.

This study measured cervical length (CL) at 11-14 and 23-24 weeks of gestation to determine the potential value of difference between measurements for prediction of preterm delivery (PTD). Sonographic measurement of CL at 11-14 weeks gestation and the difference between CL in the first and second scans are not reliable predictors of PTD. Cervical length at 21-24 weeks in low-risk women was predictive of PTD. Once short CL was observed, risk of preterm delivery was greatly increased.

Objectives: To determine the relationship between uterine artery Doppler ultrasound indices and development of pre-eclampsia, small-for-gestational age (SGA) and gestational age at delivery.
Methods: Retrospective, multicentre study, uterine artery Doppler assessment was carried out at18 + 0 to 21 + 6 weeks' gestation in unselected women with singleton pregnancies, attending for routine antenatal care in a primary care centre. The mean resistance index (RI), pulsatility index (PI) and its relationship to the outcome of pregnancy and birthweight centiles were evaluated.
Results: Development of pre-eclampsia overall was associated with increased resistance index (RI), pulsatility index (PI), a PI or RI multiples of the median (MoM) at or above the 75th %ile, but not the presence of a notch or a bilateral notch prior to 21 weeks. The sensitivity was 45% (95% CI 35–51) and specificity 69% (95% CI 65–69) for prediction of pre-eclampsia overall. Uterine artery RI and PI showed a significant, negative correlation with birth weight (r = −0.20, P < 0.0001) respective (r = −0.28, P < 0.001). Patients with early-onset pre-eclampsia had an increased prevalence of high uterine artery mean RI, above the 90th centile, corresponding to an increased proportion of small-for-gestational age (SGA) neonates with a birthweight below the 10th centile.
Conclusions: Uterine artery Doppler sonography fulfill all the requirements for screening in prediction of pre-eclampsia. Our results are comparable to the results of other studies.

Objectives: The purpose of this study was to establish the validity of abdominal sonographic evaluation of lower uterine segment (LUS) thickness in full term pregnancies with a single previous Caesarean section, and to assess the usefulness of measuring LUS thickness in predicting the risk of uterine rupture during a trial of vaginal birth.
Methods: Three hundred and thirty six women with a single previous Caesarean section who had an ultrasound measurement of the LUS thickness (stratified in C1,C2 and C3) in pregnancy were enrolled. Abdominal sonographic assessment of LUS was carried out within a 2 weeks of delivery. Sonographic measurements were correlated visual finding of a uterine scar at the time of the iterative Caesarean section. Receiver operating curve analyses has been used to determine the detection rate (DR) and the risk of each actual value of LUS thickness versus athin uterine scar.
Results: The detection rate (DR) as estimated by ROC curves to detect a translucent lower uterine segment (C3) was 94.5% at a false positive rate (FPR) of 15%. The correspondent cut-off value was 2,5 mm. Univariate logistic regression analysis revealed that full LUS thickness of < 2. 5mm was the only factor to be correlated with translucent lower uterine segment (C3) (8.8% vs 0%;P=0,02).
Conclusions: Full LUS thickness of < 2,5 mm is associated with a higher risk of uterine rupture.

Intraoperačné komplikácie v zmysle poranení brušných orgánov sú sprievodným javom operačnej liečby v gynekológii, čo vyplýva z anatomických pomerov a topografických vzťahov reprodukčných orgánov k močovému traktu, ku gastrointestinálnemu traktu, k cievam a nervovým zväzkom.

Opisuje koncepcie chirurgických postupov, skórovacie systémy a manažment tehotných žien s poraneniami.

Monitorovanie saturácie kyslíkom významne zlepšilo zdravotnícku starostlivosť v mnohých oblastiach, vrátane anesteziológie, intenzívnej medicíny a starostlivosti o novorodenca.

Jednoznačný prínos peritoneálnej cytológie pri tomto ochorení z hľadiska výberu ďalšej liečby a stanovenia prognózy bude potrebné overiť v randomizovaných štúdiách.

Počas posledných dvadsať rokov viaceré štúdie dokázali prognostický význam viacerých parametrov zahŕňajúcich histologický typ, stupeň diferenciácie - grade, štádium, hĺbku invázie myometria, vaskulárnu inváziu a postihnutie cervixu. Tento prehľadný článok prezentuje najdôležitejšie prognostické faktory karcinómu endometria.

Karcinóm endometria predstavuje v súčasnosti najčastejší gynekologický malignóm. Podľa mechanizmu patogenézy možno ochorenie rozdeliť na dva základné typy, estrogéndependentný endometrioidný typ a na stimulácii estrogénom nezávislý serózny typ. Štandardnou diagnostickou metódou je kyretáž maternice. Rozhodujúcou stránkou terapie je operačná liečba. Adjuvantná liečba je závislá na histologickom náleze a mala by byť individualizovaná. Článok prináša prehľad súčasného stavu v pohľade na patogenézu, diagnostiku a liečbu karcinómu endometria.

Aim: To evaluate the routine surgical treatment of endometrial cancer in Slovak Republic in relation to current international recommendations.
Methods: A retrospective study based on a questionnaire was undertaken. Data on surgical and post-operative adjuvant therapy of endometrial cancer patients in Slovakia in 2001 were collected, assesed and validated for good clinical practice.
Results: We presented data of 298 cases of endometrial adenocarcinoma from 48 of 66 Slovak gynecologic departments. Laparotomy with hysterectomy and bilateral salpingo-oophorectomy was performed in 280/298 patients. Peritoneal washings were examined in 41/298 cases. Lymphadenectomy (pelvic and/or para-aortic) was performed in 52/298 of the women. Malignancy remained undiagnosed in 29/298 of the cases until it was detected by histological investigation of extirpated uterus.
Conclusion: The study demonstrates that surgical management of endometrial cancer is far from optimal. It seems necessary to restrict treatment to the centres with gynecologic oncologists trained in pelvic surgery.

Background: Gynecologic cancers metastatic to bone are rare. Endometrial carcinoma usually presents with vaginal bleeding.
Case Report: A 67-year-old woman presented with pain, erythema and swelling of the right foot and no history of postmenopausal bleeding. Biopsy revealed primary endometrioid carcinoma metastatic to the calcaneus, talus and metatarsal bones. Lower leg amputation, total abdominal hysterectomy, bilateral salpingo-oophorectomy and pelvic lymph node sampling were performed. Postoperatively the patient received cisplatin with adriamycin and megestrol acetate and is alive with no evidence of disease 20 months after the diagnosis.
Conclusion: Endometrial carcinoma can present as a metastatic lesion of bone.

Background: The risk of acid aspiration and its subsequent development into Mendelson’s syndrome is still an important issue of all clinicians involved in the care of the pregnant women. Mendelsohn´s syndrome represents a significant cause of maternal mortality in patients undergoing caesarean section (1). In view of the increasing frequency of caesarean sections in Slovakia, we conducted a retrospective study of caesarean sections in Slovakia over a one-year period from January 1, 2005 to December 31, 2005 with the aim of evaluating acid aspiration prophylaxis practice.
Method: Questionnaires on the practice of acid aspiration prophylaxis for caesarean section were mailed to 66 Slovak departments of obstetrics. If acid aspiration prophylaxis was used, further questions were asked relating type of drugs, dose and route of administration.
Results: A response rate of 92% was achieved. The 61 completed replies in this survey represents 93,3% of all Slovak deliveries in 2005. Pharmacological acid aspiration prophylaxis is routinely used in 65,5% of departments before caesarean section. H₂ blocking drugs administered venously are preferred for acid aspiration prophylaxis.
Conclusion: Because of rising rate of caesarean sections acid aspiration prophylaxis deserves more widespread use in obstetric anesthesia.

Primary appendiceal malignancy metastatic to the ovaries is a condition that may mimic advanced stage ovarian cancer. This condition is rarely diagnosed preoperatively. A 52-year-old woman referred to our institution for presumed advanced stage of ovarian cancer was found to have primary appendiceal adenocarcinoma metastatic to ovaries at laparotomy. We describe the clinical course of the patient. It is important for the gynecologist-oncologist to include tumors of the appendix into the differential diagnosis of any case of ovarian tumor.

Cieľ: Prezentovať prípad masívneho postpartálneho krvácania po cisárskom reze s úspešnou liečbou použitím rekombinantného faktora VIIa.
Pracovisko: Gynekologicko-pôrodnícka klinika, Fakultná nemocnica Nitra, Slovensko.
Kazuistika: Autori uvádzajú prípad 29-ročnej secundigravidy s ťažkým postpartálnym krvácaním po cisárskom reze v dôsledku atónie uteru. U pacientky sa rozvinul hemoragický šok s diseminovanou intravaskulárnou koaguláciou (DIC). Liečba uterotonikami, prostaglandínmi a následnou hysterektómiou nebola úspešná v snahe o kontrolu difúzneho krvácania.

Background: Patients with endometriosis rarely have a serum CA 125 concentration >100 IU/mL. A raised plasma level of D-dimer indicates active fibrinolysis, either secondary to clot formation or primarily activated. This condition is seldom diagnosed in patients with endometriosis.
Case report: A 53-year-old woman was referred to our institution for acute abdominal pain. Laparoscopic surgery revealed a large ovarian cyst with rupture on the left side. Preoperative laboratory tests detected high serum CA 125 and D-dimer levels. Adnexectomy was performed, resulting in a sharp decrease in serum CA 125 and D-dimer concentration. We describe the clinical course of the patient.
Conclusion: Rupture of a large ovarian endometrioma can lead to a high serum concentration of CA 125, a condition which, in addition to the detected pelvic mass, may mimic a malignant process. The increased D-dimer plasma level indicated that a ruptured endometriotic cyst can induce coagulation reactions.

Metóda: Prehľad literárnych zdrojov z databázy Medline s využitím kľúčových slov vulvar cancer, surgical therapy, radiotherapy, sentinel node so zameraním na diagnostiku a primárnu liečbu.
Výsledky: Operačná liečba použitím širokej lokálnej excízie a uni/bilaterálnej inguinálnej lymfadenektómie prostredníctvom samostatných rezov je u včasných štádií skvamocelulárneho karcinómu vulvy základným liečebným prístupom.

The purpose of this study was to conduct a clinical and pathologic review of endometrial cancers diagnosed in women aged younger than 45 years to better identify the prognostic factors for this subgroup of women. We retrospectively evaluated the clinical history, treatment, and follow-up of patients with histologically confirmed endometrial cancer treated in Faculty Hospital Nitra, Slovakia from 1993 to 2003. Data were abstracted regarding tumor histology, grade, age, parity, stage, diabetes, use of oral contraceptives, body mass index (BMI), and survival. One hundred seventy-three patients with endometrioid histology were divided into two groups: younger group (age 45, n = 153). Patients with high-risk histology (clear cell or serous papillary) were excluded from the study. Twenty patients less than or equal to 45 years of age received treatment for endometrial cancer: stage I, 16 (80%); stage II, 2 (10%); stage III, 1 (5%); and stage IV, 1 (5%). Tumors were well differentiated in 12 (60%), moderately differentiated in 6 (30%), and poorly differentiated in 2 (10%). Age ranged from 28 to 45 years (mean 37), with mean BMI 35.8 +/- 9.4. At the end of study period, 17 (85%) were alive with no evidence of disease and 3 (15%) had died of recurrent disease. We conclude that patients less than or equal to 45 years of age have better survival compared to older patients. Deeper myometrial invasion was significantly associated with age greater than 45 years. Majority of young patients with endometrial cancer were obese and nulliparous.

Endometrial carcinoma is the most common malignancy of the female genital tract in industrialized countries, and occurs predominantly after the menopause. Although most endometrial carcinomas are detected at low stage, there is still a significant mortality from the disease. In postmenopausal women, prolonged life expectancy, changes in reproductive behavior and prevalence of overweight and obesity, as well as hormone replacement therapy use, may partially account for the observed increases of incidence rates in some countries. In order to improve treatment and follow-up of endometrial carcinoma patients, the importance of various prognostic factors has been extensively studied. The identification of high-risk groups would make it possible to avoid unnecessary adjuvant treatment among patients with a good prognosis. Over the past few decades, several studies have demonstrated the prognostic importance of different parameters including lymph node status, histological type of carcinoma (serous carcinoma and clear cell carcinomas are poor prognostic types), histological grade, stage of disease, depth of myometrial invasion, lymphovascular space involvement and cervical involvement. Other factors currently being investigated are estrogen and progesterone receptor status, p53 status, flow cytometric analysis for ploidy and S-phase fraction, and oncogenes such as HER-2/neu (c-erbB-2).

Bakteriálna kontaminácia operačnej rany je pomerne častá v gynekológii a pôrodníctve. Použitie antibiotickej profylaxie znížilo výskyt, ale nezabránilo úplne vzniku postoperačných infekcií. V diferenciálnej diagnostike treba zvážiť jednoduchú infekciu, absces, nekrotizujúcu fasciitídu a septickú panvovú tromboflebitídu. Absces prípadne nekrotizujúca fasciitída vyžadujú invazívnu operačnú revíziu a širokospektrálnu antibiotickú terapiu. V prípade jednoduchej infekcie a septickej panvovej tromboflebitídy by mala postačovať antibiotická liečba.

Objective: The aim of this study was to evaluate properative serum levels of CA-125 for the prediction of advanced stages of endometrial cancer.
Method: Retrospective study evaluated 114 women with pathologically proven endometrial carcinoma treated in our institution that had preoperative serum CA-125 levels between January 1990 and December 2005. The association of preoperative serum CA-125 with a different histopathologic factors was evaluated. Statistical analysis was performed using Mann-Whitney U test and Kruskal- Wallis test and a logistic regression.
Results: Elevated serum CA-125 levels were significantly correlated with advanced-stage disease, lymph node metastases, increased depth of myometrial invasion (P < 0,01). Similar correlation was seen betwen levels of CA 125 and positive peritoneal washing cytology, distant metastases and highest histologic grade. Multivariate analyses using logistic regression showed that lymph node metastases had the most significant effect on the elevation of preoperative serum CA-125 levels.
Conclusion: The preoperative serum CA-125 level appears to be a significant independent predictor of advanced-stage disease and lymph node metastasis. Therefore, preoperative serum CA-125 may be a useful tool, in the clinical setting, for optimal individualized patient management. A CA 125 level should be included as a part of the preoperative workup for all patients with endometrial cancer.

Endometrial cancer is the most common gynecologic malignancy, with an incidence of 25 cases per 100,000 women in Slovakia in 2000. A great variety of uterine and extrauterine prognostic factors (including histologic type, stage, grade, depth of myometrial invasion, vascular invasion, lymph node involvement, hormone receptor status, DNA index, peritoneal cytology, p53, c-erb-B2 (HER2/neu)) have been described and evaluated in literature. The aim of this study was to determine and quantify overall survival and recurrence-free interval of endometrial cancer patients over the 15 years period to evaluate multivariate independent prognostic factors in the prognosis of patients with carcinoma of the endometrium.

Objective: The purpose of this study was to conduct a clinical and pathologic review of endometrial cancers diagnosed and surgically treated in our institution to evaluate results of treatment in relation to current international recommendations.
Methods: We retrospectively evaluated the clinical history, treatment and follow up of patients with histologically confirmed endometrial cancer treated in Faculty Hospital Nitra, Slovakia from 1990 to 2005. Data were abstracted regarding tumor histology, grade, age, parity, stage, diabetes, use of oral contraceptives, BMI, survival and treatment modalities including surgery, radiation therapy, chemotherapy, hormonal therapy, and combinations thereof.
Results: 139 patients received surgical treatment for endometrial cancer: stage I –95 (68,3%), stage II – 15(10,8%), stage III – 23 (16,6%) and stage IV – 6 (4,3%). Tumors were well differentiated in 87(62,6%), moderately differentiated in 32 (23%) and poorly differentiated in 20 (14,4%). There were 45 (32,4%) premenopausal patients and 94 (67,6%) postmenopausal.
Conclusion: In multivariate statistical analysis we identified FIGO stage, tumor type, tumor grade, nodal status and depth of myometrial invasion as independent prognostic factors for overall survival, and FIGO stage, nodal status, and tumor grade as independent prognostic factors for recurrence-free interval.
